WAIVER OF LIABILITY/RELEASE

I hereby warrant that I am in good physical condition and have no disability or impairment preventing me from participating in the North American Karate and
Fitness Championships (“NAKF Championships”). Further, I understand and acknowledge that my participation in the NAKF Championships will involve
strenuous physical activity and physical contact, and may be hazardous and may result in personal injury. I acknowledge that I am participating the NAKF Championships at my own risk.

Recognizing and assuming the risks involved therein, I hereby waive liability as to and relinquish all rights that I have now or may have in the future against the North American Karate and Live Casino at Westmoreland Mall, Master Ron Robb (“Master Robb”), Live Casino, Westermoreland Mall, and all their respective
partners, affiliated companies, officers, directors, agents or employees, in connection with my participation the NAKF Championships. In addition I agree to indemnify and hold North American Karate and Fitness, Master Robb, and Live Casino harmless from and against any and all claims, demands, fines, suits, actions, orders or damages of any kind which may arise or result out of or from my participation the in the NAKF Championships.

I also release and forever discharge North American Karate and Fitness, Master Robb, and Live Casino its respective partners, affiliated companies, officers, directors, agencies, and employers from any and all claims which I may hereafter have for lost of life, bodily and personal injury, medical expenses, property
damage and all other incidental expenses or losses which may arise as a result of my participation in the NAKF Championships.

JUNIOR CONSENT FORM AND WAIVER OF LIABILITY/RELEASE

I hereby grant permission for my child to participate in the North American Karate and Fitness Championships (“NAKF Championships”). I hereby warrant that my child is in good physical condition and has no disability or impairment preventing him/her from participating in the Championships. Further I understand and acknowledge that my child’s participation in the Championships will involve strenuous physical activity and physical contact, and may be hazardous and may result in personal injury.

Recognizing and assuming the risks involved there in, I hereby waive liability as to, and relinquish all rights that I have now or may have in the future against North American Karate and Fitness, Master Robb, and Live Casino, and all of their respective partners, affiliated companies, officer, directors, agent or employees, in connection with my child’s participation in the Championships.
